In this first section of the Core Framework, we start with the central tension in benzodiazepine prescribing: these medications can provide unusually rapid and meaningful relief, while also creating risks and responsibilities that can persist long after the original prescription.

We also introduce the approach used throughout Benzodiazepines in the Grey Zone: where the evidence is clear, we’ll use it; where it becomes limited, I’ll be explicit about the clinical heuristics I use in practice.

In this section:

  • Why benzodiazepines can be both highly effective and clinically difficult
  • The role of rapid relief in anxiety, distress, and functioning
  • Dependence, withdrawal, sedation, cognitive and psychomotor effects, and co-use risk
  • How relief can sometimes reinforce avoidance or weaken self-efficacy
  • The difference between a medication helping and a medication being risk-free
  • Three questions to return to throughout benzodiazepine prescribing: What problem is it solving? What new problems might it be creating? What responsibility am I assuming by prescribing it?
